Just as the schematic is pretty darn simple, the layout is similarly just as simple. Basically I just let the auto-router work its magic which ended up giving me some 90 degree turns, but that's acceptable for what I'll be using this board for. The plane around the board is connected to both grounds.
Feel free to download the Eagle project archive above and load up the schematic and layout on your computer. There's really nothing special to it. A nice breakout board, from dual row IDE to single row headers for a breadboard!
